Please enter ADX NIN, DFM NIN or the shareholder Code/Number of the company for which you appear to have not received the dividends, in addition please enter the name as shown in your passport/Emirates ID or family book or as you registered with ADX/DFM/ the Company.

Note:

• The investor number / shareholder number must match the name registered in the company's Share Book or ADX or DFM.

• ADX NIN should be 15 Digits.

• DFM NIN Should be 10 Digits.
